If your student has life-threatening allergies for which an Epi-Pen may be required, please print out and complete the following forms:

The forms with the Epi-Pen must be returned to the school nurse.  The forms are valid for the entire school year.


If your student has asthma and may need an inhaler during school hours, please print out and complete the following form:

The form with the inhaler must be returned to the school nurse.  If permitted by your physician, your student may carry and/or self-administer his/her own inhaler, however the nurse must still have the Asthma Treatment Plan (signed by physician and parent) on hand in the health office.  Forms are valid for the entire school year. 


If your student will need any prescription or over the counter medication administered during the school day, please complete the following form:

The form with the medication must be returned to the school nurse.  The medication must be in the original container (and appropriately labeled by the pharmacist if it is a prescription medication). 


Health Screenings are done on all students unless we have obtained the required information from the sports physical completed June 1st or after for the new school year.

  1. Height, weight and blood pressure will be done on all students.
  2. Hearing screenings will be done on all students in grade 7.
  3. Vision screenings will be done on all students in grade 8.
  4. SCOLIOSIS screening will be done on students in grade 7.  Scoliosis is a disease that causes the spine to curve.State law NJSA 18A:40-4.3 requires screening for scoliosis to be conducted biennially for students between the ages of 10 and 18.If you prefer to have your child examined by your own health care provider, at your expense, a note from your physician stating that your child has had a scoliosis examination must be submitted to the school nurse by December 1, 2020.Scoliosis screenings will likely be scheduled for after the winter break for the 2020-2021 school year.
  5. State guidelines are followed to monitor immunization records on all students.


  • In order for your child to participate in any sport in the South Plainfield School District they must have a physical examination done within 365 days of the first practice of the season. The state mandated pre-participation physical evaluation (PPE) form is the only accepted form and must be completed in full by the examining healthcare provider and parent. If the physical examination was conducted 90 or more days prior to the first practice and a PPE form is already on file in the health office, then a Health History Update Questionnaire (HHQ) must be completed and signed by the parent/guardian.
  • **Please note that the pre-participation physical evaluation must be completed by a Healthcare Provider who has completed the Student Athlete Cardiac Assessment Professional Development Module. Their signature with the date of completion of this module is required on the bottom of page 4 of the PPE packet.
  • Any forms not completed properly or signed in all the required areas by the physician, parent and student will be returned to the student, delaying participation of the athlete.
  • According to N.J.A.C. 6A:16-2.2(h)1iv, all PPE forms must be reviewed and approved by the school physician before a student is eligible to participate.
  • Late Registration: A student who does not submit all required forms by the due date should expect a delay in the processing of paperwork and clearance for sports participation. A minimum of 72 hours for processing will be required. PPE forms submitted on the first day of tryouts/practice will not be cleared for participation until at least the third day of practice.

  •  All additional paperwork can be accessed on the Genesis Parent Portal.



All winter sport physicals AND/OR update forms must be submitted to the nurse by
November 16, 2020
Forms can be handed to Nurse Wollman, emailed or  faxed to Nurse Wollman (information below) OR  handed  in to the Middle School main office.
